Plan Information
Employees who opted for an early retirement incentive and are receiving an ongoing monthly benefit.
The following forms are available:
- Beneficiary Add/Change Form: To update or add beneficiary information and/or designations.
- Change of Address Form: To update your home mailing address
- Direct Deposit / Rollover Change Form: To update or set-up either your direct deposit or rollover account information
- Direct Deposit Rescission Form: To have your monthly check mailed directly to your home address
